Format A Survey


You can use Format A when:

  • You have 40 or less single-answer questions and each of the questions can be answered using the rating 1, 2, 3, 4 or 5 (from 'low' to 'high' or from 'bad' to 'good') , such as:

      1. Are you satisfied with our service?
      Not At All 1 2 3 4 5 Very Much So

      2. What do you think of the movie "Spider Man" ?
      So Boring 1 2 3 4 5 Love It

      (Note: You can change "Not At All", "Very Much So", "So Boring" or "Love It" to anything you want)

    and/or

  • You have 20 or less comments/feedback questions such as:

      1. Do you have any comments on the movie "Spider Man"?

Format B Survey


You can use Format B when:

  • You have 40 or less single or multiple-answer questions and each question has 10 or less options/answers for people to select (including the "Other" option), such as:
      1. What do you usually have for breakfast?
        Coffee
        McDonald
        Egg
        Toast
        Eat Nothing
        Others


      2. How many personal computers does your company have?

        Less than 10
        11 - 50
        51 - 100
        Over 100


      3. What's your salary?


    and/or

  • You have 20 or less comments/feedback questions such as:
      1. Do you have any comments on the movie "Spider Man"?

Format C Survey


You can use Format C when:

  • You want to put a quick vote on your web site, such as:

  • For Format C questions, you can have up to 10 options/answers.
